متى بنضيف لمريض ال COPD ال Roflumilast ⁉️

شفنا البوست السابق ان في حاله المريض مشى ع LAMA-LABA-ICS ولسه بيدخل ب Exacerbation
هنشوف شو نضيفله أدويه حسب ال Esinophil وال Phenotype

وعرفنا ان لو ال Eosinophil Count كان 300 فما فوق بهلحاله بندخل بال Biological Therapy
As Dupliumab، Mepolizumab

📍 أما في حاله كانت ال Eosonophil أقل من 300
وبعض المراجع اقل من 100 هنضيفله أدويه واخرى

اليوم هنشوف متى بنضيف ال Roflumilast ⁉️
عشان نضيف ال Roflumilast لازم يتوفر بالمريض تلات حاجات :

📍Chronic Bronchitis
📍FEV1 Less than 50
📍history of frequent COPD exacerbations (eg, at least two per year or one requiring hospitalization)
بهلحاله بنضيف ال Roflumilast
كيف بشتغل ؟؟
هو عباره عن Phosphodiesterase 4 Inhibitor
PDE4 Inhibitor
ال PDE مسئول عن تكسير ال cAMP , فلما نثبط ال PDE
هيزيد ال cAMP في ال lung tissue
ووجدا ان ال cAMP بيلعب دور ضد ال Inflammation

🔸️Anti-inflammatory effects include suppression of cytokine release and inhibition of lung infiltration by neutrophils and other leukocytes. Pulmonary remodeling and mucociliary malfunction are also attenuated

Dose :
250 mcg once daily for 4 weeks, followed by 500 mcg once daily.

⛔️ ممنوع اعطاءه لمرضى ال Moderate-sever Hepatic Impairment ..
وممنوع اثناء الرضاعه

🔸️من أشهر ال SE لل Roflumilast:
Insomnia، weight loss , Psychiatric event,
وبيتداخل مع الادويه اللي تعتبر CYP450 Inducer
و ال CYP3A4 , CYP1A2 Inhibitor

لو مريض عنده abscess وعمل Culture وطلعت Gram + Staphylococcus
وكان من أحد الخيارات ال Sensitivitie هو ال Clindamycin

📍 قبل ما نقرر ان ال Clindamycin هيشتغل ويكون effective
بنشوف هل ال Erythromycin طالع بالمزرعه ؟؟؟
لو كان ال Erythromycin طالع Resistance
فبنسبه كبيره ال Clindamycin مش هيكون effective

لان البكتيريا لما تعمل mutation وتصير Resistance لل Erythromycin هتكون بردو Resistance لل Clindamycin
حتى لو كان Sensitivie in Vitro ⛔️

📍والسبب ان ال Erythromycin يعتبر 50S Inhibitor
وال Clindamycin بردو بيشتغل عنفس ال target اللي هو ال 50S

فال mutation هتصير للدوائين طالما بيشتغلو ع نفس المكان ...

لكن ممكن يكون في سبب تاني لل Resistance زي مثلا ال
Efflux pump
📍فال test الفاصل في الموضوع هو ال D test ..

🔸️Positive D-Test: If the test is positive, it means the Erythromycin resistance gene can "turn on" Clindamycin resistance once the patient starts treatment. In this case, Clindamycin should NOT be used, as it will likely lead to treatment failure.

🔸️​Negative D-Test: If the test is negative, it indicates the resistance to Erythromycin is caused by a different mechanism (like an efflux pump) that doesn't affect Clindamycin. In this case, Clindamycin is safe and effective to use.

من أحد المضادات الحيوية اللي بيتركز بال Urine وشغله مقتصر على ال Urinary Tract ويُستخدم لل Simple Cystitis هو ال Nitrofurantoin

📍 يعتبر bactericidal ، بيثبط بروتينات ال
Bacterial Ribosome
بالتالي البكتريا مش هتقدر تصنع بروتيناتها ولا ال Cell wall
ولا ال DNA&RNA

📍يؤخذ مع الأكل عشان نزود امتصاصه ونقلل ال GI Upset

📍في شكلين لل Nitrofurantoin:
🅰️ Nitrofurantoin Monohydrate/Macrocrystal ( Macrobid)
ويُعطى مرتين باليوم ، وممنوع نفتح الكبسولة ونذوبهاا

🅱️ Nitrofurantoin Macrocrystal (Macrodantin )

و بيُعطى أربع مرات في اليوم ، وممكن نفتح الكبسوله ونذوبها بشرط نستخدمها Immediately

📍 Indication & Doses :

1️⃣️ يستخدم لحالات ال Acute Uncomplicated Cystitis
بجرعة 100mg مرتين في اليوم من ال Macrobid
او بجرعة 50-100mg اربع مرات في اليوم من ال Macrodantin
بنعطيه لمده 5 ايام لل female , و 7 ايام لل male

وبالأطفال يعطى بجرعة 5-7 مجم لكل كجم وبنقسمها على مدار ٦ ساعات ( Macrodantin )
او بجرعة 100mg مرتين من ال Macrobid

⛔️ لا يستخدم في حالات ال pyelonephritis
⛔️ وما بنبدأ فيه empiric لو المريض ماخده قبل 3 شهور

2️⃣️ في حالات ال Asymptomatic Pregnant Bacteriuria
لو كان ال Count لنمو البكتريا أكتر من 100.000CFU بنعطي مضاد حتى لو كانت ال Asymptomatic ...
يُعطى بجرعة 100mg مرتين باليوم من ال Macrobid

⛔️ ممنوع استخدامه من الأسبوع 38 للاسبوع 42 ، او خلال الولاده ، وممنوع استخدامه بالطفل اللي عمره تحت الشهر

لانه الneonate بيكون عنده immature erythrocyte enzyme system ف مش هيعرف يتخلص من ال Nitrofurantoin وبالتالي في احتماليه كبيره انه يعمله
Hemolytic Anemia

⚠️ بال 1st trimester يفضل عدم استخدامه الا لو ما في عنا خيار غيره ..

3️⃣ يستخدم ك Prophylaxis لل Recurrent Cystitis
📍في حالات ال Recurrent simple Cystitis في غير الحوامل
ممكن نستخدم ال Nitrofurantoin لمدة تتراوح من 3ل12 شهر ، بجرعة 100mg قبل النوم
وبالأطفال بجرعة 1او 2mg/kg مره واحده قبل النوم

⚠️ ال Prolong use لاكتر من 6 شهور غير مفضل لانه اتسجلت حالات دخلت بالاعراض التاليه من الاستخدام المطول :
Diffuse Intersitial pneumonitis , Pulmonary fibrosis
Chronic Hepatitis, Neuropathy

⚠️ في الأطفال اقل من 24شهر بنستخدمه ك Prophylaxis فقط بالحالات التالية :
Grade 3-5 Reflux or with Recurrent febrile UTI

📍 في حالات ال Postcoital prophylaxis
Cystitis Related to Sexual intercourse
يُعطى بجرعة 100mg خلال 2hr من ال Intercourse
او جرعة 50-100mg من ال Macrodantin

⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️

📍 لا يفضل استخدام ال Nitrofurantoin بال Geriatric
لانه ممكن يسبب Pulmonary Toxicity او Hepatic Toxicity , او Peripheral Neuropathy
خاصة مع ال Long term Use

📍بننبه المريض انه ممكن يعمل Urine discoloration

📍لو المريض ماشي ع Eplerenone وهياخد Nitrofurantoin, بنتابع ال K level
Nitrofurantoin may enhance the hyperkalemic effect of Eplerenone.

⛔️ When Nitrofurantoin is Contraindicated ;

الحالات التالية ممنوع تاخد Nitrofurantoin :

🔸️ال neonate اللي أعمارهم أقل من شهر ( والأم اللي بترضع طفل اقل من شهر يفضل تبعد عن ال Nitrofurantoin)
🔸️حالات ال anuria او oliguria ، او ال CrCl اقل من 60
( وبعض المصادر بتسمح استخدامه لحد CrCl 30 )
🔸️ لو المريض عنده History سابق ل :
Cholestatic Jaundice or Hepatic Dysfunction
مع استخدام ال Nitrofurantoin ..
🔸️ ممنوع للحوامل من الأسبوع ال 38 للأسبوع ال 42
🔸️ ممنوع في حالات ال G6PD
🔸️ممنوع اعطاءه لأم مرضعة ل premature Neonate

📍Nitrofurantoin in doses >10 mg/kg/day may cause spermatogenic arrest and decrease sperm count. This was observed in some males treated for 2 weeks; sperm counts returned to normal between 13 and 32 weeks after therapy was discontinued.
Consider avoiding use in patients planning to father a child

أحد الأدوية اللي تستخدم ك add on therapy في ال COPD
هو ال Ensifentrine
بينتمي لمجموعة ال PDE-3&4 Inhibitor
ال PDE3 مسئول عن تكسير ال cAMP و ال cGMP
وال PDE4 مسئول عن تكسير ال cAMP
فلما نثبط ال PDE3&4 , هيتراكم ال cAMP&cGMP
وبالتالي رح يصير عنا more Bronchodilatation

Inhibition results in accumulation of intracellular levels of cAMP and/or cGMP, resulting in various downstream signaling effects.

متى بنضيفه ؟؟
📍 لو مريض ال COPD ماشي ع dual Bronchodilators
ال LAMA+ LAMA ولسه عنده
persistent shortness of breath or exercise limitation
هنا ممكن نضيفله ال Ensifentrine Oral Inhalation
يُعطى ك Nebulization بجرعة : 3mg كل 12ساعه

من أشهر ال SE إله هي ال neuropsychiatric event :

An increase in psychiatric adverse reactions (eg, anxiety, depression, insomnia, suicide attempt) have been reported. Before initiating treatment, carefully weigh the risks and benefits of treatment in patients with a history of depression and/or suicidal thoughts or behavior ..
