DS NURSING ONLINE CLASS TIPS Postpartum Hemorrhage (PPH) 👉Definition Excessive bleeding following childbirth. •WHO definition: Blood loss >500 ml after vaginal delivery or >1000 ml after cesarean section within 24 hours of birth. 👉Classified as 👇 Primary PPH: Occurs within first 24 hours. 👇 Secondary PPH: Occurs after 24 hours up to 12 weeks postpartum. 👉Causes (4 T’s) 1. Tone – Uterine atony (most common cause). 2. Tissue – Retained placenta or clots. 3. Trauma – Genital tract lacerations, uterine rupture, hematoma. 4. Thrombin – Coagulation disorders (DIC, clotting abnormalities). 👉Risk Factors 1.Prolonged labor or rapid labor 2.Over distended uterus (multiple pregnancy, polyhydramnios, large baby) 3.Induction/augmentation with oxytocin 4.Previous PPH 5.Operative delivery (forceps, C-section) 👉Clinical Features ✓Excessive vaginal bleeding ✓Signs of hypovolemic shock (pallor, tachycardia, hypotension, restlessness) ✓Boggy, soft uterus (if atony) 👉Management 👉Immediate Actions:- Call for help, assess vitals, IV access with fluids, oxygen. Massage uterus to stimulate contraction. Administer uterotonics (Oxytocin, Misoprostol, Ergometrine, Carboprost). Explore and repair genital tract trauma. Remove retained products manually if needed.
